Policy

Memorial Hospital of Lafayette County will bill your insurance company on your behalf if you have provided us with your insurance information at the time of registration. If you did not have your insurance information with you, we ask that you please mail or fax a copy of your insurance card to us within 10 days of your appointment to ensure that your claims will be filed timely with your insurance company.

Insurance Coverage

Here at Memorial Hospital of Lafayette County, we participate in a variety of health insurance plans. Please check with your individual insurance plan to see if the provider you are scheduled with is in network with your plan. Every health plan is different, and each may require a different copay, deductible, and/or coinsurance. If you have questions regarding your specific plan benefits or network providers, you can contact your insurance company by viewing the information on the back of your insurance card or by visiting their website.

Payment

All charges not paid by your insurance company, including coinsurance and deductible amounts, are expected to be paid within 30 days of billing, unless you contact us to establish payment arrangements. Payments can be made online or in person by cash, check, money order, and credit or debit cards. Cards accepted include American Express, Discover, MasterCard, and Visa.

Financial Assistance

A Financial Counselor is available to discuss payment options with you and, if necessary, set up mutually agreeable payment plans for your medical bill. If you have no insurance or a high deductible insurance plan, our Financial Counselors will also review with you programs for which you may qualify and can assist in the application process.

Memorial Hospital of Lafayette County will make available, upon request, an application for our Patient Financial Assistance Program to any patient or responsible party. If you apply for financial assistance, you will be asked to supply a copy of your most recent federal tax return and a recent check stub (if applicable). This will assist us in determining if you qualify for this program. Federal Poverty Guidelines are used to determine eligibility. Elective services provided by the hospital are not eligible for this program.
